Welcome to Active Care Partnerships

At Active Care Partnerships we provide care and support for a number of different needs in a variety of different settings.

With forty two sites throughout England and Scotland our services include residential, nursing and supported living services. We provide care and support for individuals with Physical Disabilities, Learning Disabilities, Autistic Spectrum Disorder, Dementia, Mental Health issues and Acquired Brain Injuries.

As part of the Southern Cross Group, the largest independent provider of care homes in the UK, we have substantial resources at our disposal to achieve our goals. We have ambitious growth plans to create new centres which can deliver care and support to our very high standards.

We work closely with our partners in Local Authorities, NHS Trusts and Primary Care Trusts to ensure that we share the latest best practice for the benefit of our service users. An example of this is our Best Practice Group for supporting people with Autism. With expert input from our own Autism Consultant, this group means we can apply the latest thinking to our practices and support.

About our Care and Support

Our objective is very simple. We want the individuals we support to achieve the best quality of life that they can within a community setting. Our programmes of support, therapy and activities are designed to improve or maintain levels of independence and physical, psychological and social well-being.

Each individual in our care has their own personal care plan designed to achieve these objectives and help them reach their full potential. Input from friends, family and health professionals is always sought to ensure these plans truly reflect the needs and wants of the service user. We firmly believe in the value of a holistic approach to care. Independent living skills, meaningful activities and social interaction are a cornerstone of our approach. Our staff are very experienced in listening, understanding and responding to the needs of service users.

It is the aim of each and every one of our centres to provide an environment in which service users can truly feel safe and comfortable whilst maintaining their independence. We treat each service user as an individual and create a friendly and relaxing environment which helps service users feel at home.

Many of our homes are purpose built with dining and lounge facilities where service users and their friends and families can relax. Individual bed rooms are tastefully furnished with plenty of scope for each service user to make their rooms their own.

All service users should be entitled to the very best services at all times. To ensure this happens, Southern Cross has invested heavily in the training and development of their staff.

Our team of people are central to the service we provide. We know the best care and support comes from a motivated and knowledgeable team. We are totally committed to giving our people the training, opportunities and support they need to help them deliver the standards of care we demand. Our operational team is one of the most experienced in the UK. The skills, knowledge and experience of the people working in the centre are vital to the quality of the service we are able to provide.

The drive for quality is also taking place on another level – over 100 homes within the Group have currently achieved the ISO Quality Mark 9001-2000. It is the company’s intention that all homes should achieve this accreditation.
