CareMax is a healthcare management consulting company committed to working in partnership with organisations to develop bespoke solutions that improve health outcomes. Our team of interdisciplinary healthcare professionals have designed and implemented case management programmes both in the United Kingdom and abroad. Two of our Operational Directors, Jane Melvin and Jayne Molyneux are senior nurses that led the development of a pioneering NHS case management service targeted at both older people and cancer patients at the Castlefields Health Centre in Cheshire. The service was later developed into a model promoted by the Department of Health. Additional members of the CareMax team contributed to the development of curriculum, training and mentoring case management programmes within the NHS. Other team members bring international expertise in delivering change and project management within national health systems, public and private partnerships, multi-hospital systems, insurance carriers and employee groups.

Our fundamental approach is to provide practical solutions to support and strengthen the delivery of care for primary care organisations. CareMax projects begin with a detailed organisational review of problems and issues to be addressed, an in-depth understanding of the local situation, and agreement on the objectives for improving health outcomes and service delivery targets. In close consultation with service stakeholders, CareMax will develop a roadmap for service re-design and a detailed implementation plan for delivery of new services and interventions. The level of consultancy support is influenced by the resources and the expertise that each client has available to implement changes within their organisations. CareMax solutions may include bespoke workshops, staff education and training programmes, mentoring, and outsourcing of case management programmes.

CareMax has provided support within recent projects within primary care organisations to transform raw patient data into practical day-to-day decision making tools to improve patient care through effective resource allocation and targeted interventions. This information has helped commissioners and health management at PCT, Locality and GP practice levels identify where case management and community nursing support should be allocated and/or services redesigned. CareMax provides support in developing the underpinning processes and protocols that are essential to the effective functioning of a case management service, and facilitation to assist with the engagement of local service stakeholders that are key partners in delivering a successful integrated care approach.

Our training programmes focus on promoting an understanding of case management, both for case managers themselves and for those working within a case management framework. Through mentoring of staff via one-on-one reviews with our expert case managers, and through group work in problem solving workshops, we ensure that new learning is applied to practice. CareMax also provides case management support services in the field to complement and supplement existing provision. This may take the form of support for new case managers in managing complex cases, or providing temporary or permanent case managers within local teams to help address a wider geographical need, or to deliver expertise in specific long term conditions or disease processes.

CareMax services are commissioned by PCT’s, GP Commissioning Groups, individual practices and Acute Care Trusts that wish to develop a case management approach to better achieve integration with primary services.
